Effectiveness of Paradox Therapy on Alexithymia and Post-Traumatic Stress in Women Affected by Extramarital Relationships

This study aimed to determine the effectiveness of paradox therapy in reducing alexithymia and post-traumatic stress in women affected by extramarital relationships. This quasi-experimental study employed a pretest–posttest design with a control group and a two-month follow-up. The statistical population comprised women affected by extramarital relationships who attended psychology clinics affiliated with the Welfare Organization in Tehran from February to March 2025. Thirty eligible women were selected through purposive sampling and randomly assigned to an experimental group (n=15) and a control group (n=15). The experimental group received a six-session paradox therapy intervention based on Besharat’s protocol, whereas the control group received no intervention. Data were collected using the Toronto Alexithymia Scale, the post-traumatic stress questionnaire, and the Attitudes Toward Infidelity Scale. Data were analyzed in SPSS version 25 using repeated-measures analysis of variance and Bonferroni post-hoc comparisons. The time×group interaction was significant for alexithymia (F=52.189, p<0.001, η²=0.651) and post-traumatic stress (F=30.130, p<0.001, η²=0.518). In the experimental group, alexithymia significantly decreased from pretest to posttest (MD=16.400, p<0.001) and follow-up (MD=15.867, p<0.001). Post-traumatic stress also significantly decreased from pretest to posttest (MD=5.933, p<0.001) and follow-up (MD=5.467, p<0.001). Differences between posttest and follow-up were nonsignificant for alexithymia (p=1.000) and post-traumatic stress (p=0.740), indicating maintenance of treatment effects. No significant changes were observed across measurement stages in the control group. Paradox therapy appears to be an effective intervention with sustained effects for reducing alexithymia and post-traumatic stress symptoms among women affected by extramarital relationships. Accordingly, it may be considered a useful therapeutic approach in psychological and counseling services for women experiencing the psychological consequences of marital infidelity.
